Urgent Referrals
We welcome referrals, and want to make the process quick and painless for you. Our office staff are empowered to make same day appointments for any provider who requests it. If you are unsure if the patient needs to be referred the same day, or have specific questions you'd like me to follow up on, just let the receptionist know so that she can pull me out to speak with you.
Non-urgent Referrals
Again, we want to make this process quick and painless. Feel free to have your staff call us to provide the patient's name and contact information, and we will be happy to contact your patient to set up an appointment. Alternatively, you can fill out this
quick referral form, and fax it to us at 425.292.9641.
